Graham played schoolboy football with Cork sides Tramore Athletic and College Corinthians before stepping in to the League of Ireland with Cobh Ramblers in 2006. In 2007, under manager Stephen Henderson, Ramblers won the First Division title with Graham contributing 11 league goals.

At the end of 2008 the Cobh were relegated and when Henderson left to take up the reins at Waterford United in January 2009, Graham followed. Initially deployed as a centre back, he was moved up front as summer approached and he scored 17 league goals in 28 appearances for the Blues that year.

On 1st March 2010 Graham joined Cork City Foras Co-op, newly formed in the wake of Cork City's demise, and scored 18 goals in 32 league appearances for the club. In 2011, with the club having rehoisted the banner of Cork City FC, Graham added 28 goals in all competitions, including the 94th minute winner away to Shelbourne on the last night of the season which won Cork the 2011 First Division title.

The following year Graham won a contract with English League One side Preston North End, signing for the club on 30 January. He scored four times in 31 appearances for Preston before being released in the summer of 2014.

In January 2018, Graham signed back with Cork City after six years in the U.K.
