Derek is a pacey winger who played youth football with Tralee Dynamos before signing with Galway United in 2000. In three years at Terryland Park he made 45 league appearances before joining Sligo Rovers in 2003.

In 2004 he returned to Galway United. His second spell at the club brought him personal honours as United returned to the Premier Division in 2007 and O'Brien made that years LoI team of the Season as well as being voted Galway's Player of the Year.

He stayed at Galway for the first half of 2008 but signed for St Patrick's Athletic in July. He scored on his debut, coming on as a sub to bag a 65th minute winner in a 1-0 win away to Cobh Ramblers on 12 July '08. But Derek's stay at St Pats was short-lived and he left at the end of the 2008 season, returning to Galway United for a third spell in January 2009.

In February 2011 Derek moved south, signing for Tommy Dunne at Cork City where he made 19 league appearances and scored three times. But in January 2012 Derek returned to Galway, playing briefly for SD Galway during their pre-season friendlies, but on 24 February he signed for Pat Scully at Limerick FC. Derek made 16 league appearances for Limerick as they won the First Division title, earning a long awaited return to top flight football.

However, Derek didn't go to the Premier with Limerick, signing instead for First Division outfit Salthill Devon in February 2013 where he scored once (against Longford Town) in 13 league appearances before a move to Longford Town in early July. His Longford debut came in a scoreless draw with Finn Harps on 12 July and he made 10 league appearances before leaving the Midlands club at the end of the 2013 season.

Derek's next port of call was the newly formed Galway FC who he joined, under manager Tommy Dunne, on 20 February 2014 ahead of the club's debut season in the First Division. He made 11 league appearances for Galway in the first half of the season and scored one league goal, in a 5-1 win over Cobh Ramblers, but left the club by mutual consent at the end of July 2014.
